सागर/-भारत सरकार की एडिप योजना के अंतर्गत सामाजिक न्याय एवं दिव्यांगजन सशक्तिकरण विभाग द्वारा सागर जिले के दिव्यांगजनों के लिए निःशुल्क सहायक उपकरण व कैलिपर्स एवं कृत्रिम अंग प्रदान करने हेतु आयोजित किये जाने वे परीक्षण शिविर का कैलेंडर जारी किया गया है।
कलेक्टर श्री संदीप जी.आर. के आदेशानुसार शिविर का आयोजन जनपद पंचायत मालथोन में 7 जनवरी को, जनपद पंचायत राहतगढ़ में 8 जनवरी को, जनपद पंचायत शाहगढ़ में 9 एवं जनपद पंचायत केसली में 10 जनवरी को शिविर का आयोजन किया जायेगा।
एडिप योजनांतर्गत कृत्रिम अंग/सहायक उपकरण की पात्रता हेतु आवश्यक दस्तावेज।
दिव्यांगता प्रमाण पत्र जिसमें न्यूनतम 40 प्रतिशत दिव्यांगता प्रदर्शित हो व यूडीआईडी कार्ड, बीपीएल राशन कार्ड/आय प्रमाण पत्र, आधार कार्ड, समग्र आईडी, दो पासपोर्ट साईज फोटो दिव्यांगता दर्शाता हुआ, आर्थिक पात्रता में जिनकी सभी सत्रतों से मासिक आय 22 हजार 500 रुपये प्रतिमाह से कम हो वह राजस्व विभाग, सांसद, विधायक या ग्राम प्रधान द्वारा प्रदत्त आय प्रमाण पत्र प्रस्तुत कर सकते हैं।
पूर्व में आयोजित परीक्षण शिविर में जिन दिव्यांगजनों का चयन सहायक उपकरण हेतु हो चुका है उन्हें पुनः इन शिविरों में शामिल नहीं किया जायेगा।
एडिप योजना के अन्तर्गत उपकरण के निःशुल्क वितरण शिविरों का आयोजन के लिए संबंधित अनुभाग के अनुविभागीय अधिकारी (राजस्व) को नोडल अधिकारी नियुक्त किया गया है। एवं सिविल सर्जन सह-अस्पताल अधीक्षक जिला चिकित्सालय को आदेशित किया गया है कि उपरोक्त तिथि व शिविर स्थल पर जिला मेडिकल बोर्ड के विषय विशेषज्ञों की उपस्थिति सुनिश्चित करेंगे
